1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
**Person in charge paid the city cert fee and sent it to the city last month. Provided proof.
36 Rear outer opening in the food facility does not protect against the entry of insects, rodents, and other animals.
47 The following was observed:
-Static dust accumulation on the vents in the walk-in freezer.
-Food debris on the floor perimeters of the walk-in cooler and freezer.
-Ice accumulation in chest freezer and walk in freezer
-Grease on cooking equipment
-food debris under food prep table and on the shelves
53 -Gaps observed in the ceiling in the mop area.
53 Yellow stained ceiling tiles was observed during the inspection.
53 Brooms and dust pans observed not elevated on a utility rack.
54 Hood filters have an accumulation of static dust and grease.

**Person in charge removed the utensils in the food prep sink and clean the food prep sink prior to completion of the inspection.
14 Ice-cream dispenser observed to have old (hard) ice-cream accumulation.
**Person in charge cleaned and sanitized the equipment during the inspection.
37 Several (2) baking soda containers observed open and on the top shelving unit in the walk in cooler.
**Person in charge, removed from the walk-in cooler to prevent contamination of food products.
45 One ice machine observed with missing power supply front cover.
45 One cold-prep unit observed to not reach 40 degrees F but instead 50.
**Person in charge put ice pans under the pans of food, making the food reach 44 degrees F. Temperature log for every 4 hours was shown during the inspection

1 The facility's food safety certified handler lacks the City of Philadelphia food safety certificate. Observe a ServSafe certificate only. Advised facility's food safety certified handler to apply for the City of Philadelphia food safety certificate. *Application form issue today 11/9/2023.
1 The PIC did not demonstrate adequate knowledge of the Food Code as evidenced by incorrect responses to food safety questions.
*Sanitarian review with the person in charge the five food borne illnesses and hot and cold temperature of the food.
13 Observed food safety person not sanitizing probing thermometer prior to taking food temperatures.
*Health Sanitarian instructed the food safety person to sanitize probing device prior to use.*
13 - Carboard boxes with food observed stored in top of uncovered food (chicken) in the chest freezer/cooking area.
20 The food (mayonnaise, tomatoes, cheese) was held between 58 to 68 degrees F rather than 41 degrees F or below as required.
*Sanitarian advised the person in charge to remove from sale the food that was below 41 degrees F.
21 Several food items were not date marked in the walk-in cooler.
*Sanitarian advised the person in charge to provide dates for the food.
22 Time in lieu of temperature being used in the food facility to control ready to eat potentially hazardous foods without written procedures or documentation to verify disposition of food.
36 Visible physical evidence of insect activity (fruit flies) observed in the
dinning area.
37 Food is not stored at least 6 inches above the floor in the walk-in cooler.
45 The following observations were noted:
- One ice machine observed damaged during inspection.
- One reach-in freezer observed out-of-service during inspection.
