37 Consumer self service in pick up line area for ready to eat foods (apples) not provided with suitable utensils or dispensing method to prevent contamination. Students are retrieving their own fruit that is stored together in a tray rather than prewrapped or dispensed by cafeteria staff.
45 Equipment is not in good repair.: -The garbage grinder sink is not completely wired and not operating. -The sink for the garbage grinder is not working. The sink is covered with other equipment. -A large warmer is not working. -The walk-in freezer not in use and the walk-in cooler is not maintaining a temperature of 41 degrees F or lower. The ambient air temperature in walk-in cooler is 49 degrees F or higher. *The person in charge submitted repair order #6859 on 9/4/2024.
46 There is not drain rack above the 3 bay sink for the air drying of dishes, equipment and utensils.
49 The pipe for the prep sink is not directly over the drain. The pipe is half over the floor and half over the drain. This causes water to fall out onto the floor rather than down the drain. Note: The air gap must remain.
51 Staff toilet room observations.: -Static dust observed on the toilet. -The wall near the hand sink is soiled.
52 The dumpster is lacking lids. Trash observed on the ground behind the dumpster. Maintenance began cleaning this area.
54 Dim light in the walk in cooler.

45 Observations: -A hot holding unit and a garbage grinder sink not operating. -The walk-in freezer not in use and the walk-in cooler used for dry storage only. The ambient air temperature in walk-in cooler is 55°F. *The person in charge submitted repair order #6859 on 9/4/2024. -Flickering light bulb in the walk-in cooler.
45 Cutting boards observed with deep scratches and scoring not allowing for effective cleaning and sanitizing.
49 -Observed leaky pipe under the three-bay sink. Water is partially falling into a bucket. *The person in charge submitted repair order #6860 on 9/4/2024. -Leaky faucet with corrosion observed at the handwashing sink in the front serving area.
53 Observed dust on the acoustic ceiling tiles in the dining area.
54 Static dust observed on the celling vent at the women's staff toilet room.

14 The can opener had an accumulation of dried food residue on the food contact surfaces. An employee washed, rinsed, sanitized, and air dried the can opener during the inspection.
45 -The garbage grinder and sink is not operating.
47 Observations: -Food debris in the serving area hand sink. This sink was cleaned during the inspection. -Food debris and other debris on cabinet tracks in the food prep area.
49 The pipe for the indirectly drained food prep sink is not completely over the drain. Water falls partially onto the floor rather than completely over the drain.
51 Staff toilet room observations.: -Some static dust on the ceiling vent. -The hand sink was not clean to sight and touch.
52 The dumpster is lacking lids to close tightly when not in use.
53 Observations: -Residue on some floor perimeters. For example, the cooking and dining areas. -Static dust, debris, and residue on floor perimeters in the cooking, storage, and dining areas. For example, under and around cooking equipment, refrigerators, and shelves.
53 A ceiling light lens is loose in the rear area of the food prep area.
54 -The exhaust hood in the cooking area has 2 of 3 lights not working. -1 of 2 lights in the rear storage area are not working.

14 Wiping cloths observed stored in bucket with sanitizer concentration of 0 ppm in the dining area. The person in charge discarded the soap solution and prepared a fresh sanitizer solution that was tested at 200 ppm. *** Note: Wiping cloths should be stored in a sanitizer solution when not in use. ***
39 Moist wiping cloths observed lying on 3 basin sink and not stored in sanitizing solution. They were removed.
43 Take out disposable trays were stacked facing up rather than down to keep out dust and other potential contaminants. They were placed facing down.
45 -The 3 basin sink is leaking at the pipe below the wash basin. -The walk in freezer condenser is leaking in the unit causing ice to accumulate on surfaces. -The garbage grinder and sink is not operating.
45 Plastic crates are used to elevate items making inspection and cleaning difficult.
47 Observations: -Accumulated ice on the walk in freezer condsenser, shelving, and floor perimeters below.
52 The dumpster is lacking a lids to close tightly when not in use.
53 Observations: -Residue on some floor perimeters. For example, the dining area. -Static dust on floor perimeters in the cooking area.
53 Porous ceiling tiles observed in the storage room area.
54 -The exhaust hood in the cooking area does not have any working lights. -The light is not working in the refrigerator in the serving area. -The lights in the rear storage area are not working.

4 An open employee's beverage (water bottle) container was observed in the food service area. Cups with lids and a straw prevent potential contamination of hands. The bottle was moved to a table nearby.
8 The service area hand sink was lacking a waste container for paper towel disposal. A waste container was placed at this sink.
14 -Slicer has an accumulation of dried food residue on the inner food contact surfaces around the blade and is not being taken apart, washed, rinsed and sanitized at least every 4 hours. -The can opener had an accumulation of dried food residue on the food contact surfaces. The person in charge placed the can opener in the 3 basin sink wash basin and stated that she would obtain the proper brush and gloves to clean the slicer properly.
36 Visible physical evidence of rodent activity (mouse droppings) observed in the pantry area.
45 Plastic crates are used to elevate items making inspection and cleaning difficult.
45 -The garbage grinder and sink is not operating. -Hand wash only sign observed over the 3 basin sink. Note: The 3 basin sink should be used for washing dishes only.
46 Food facility lacks QAC sanitizer test strips to monitor sanitizer concentration. The person in charge had pH test strips rather than QAC test strips.
47 Observations: -Mouse droppings on lower shelves in the pantry area. -Static dust observed on the walk in units ceiling surfaces. -General debris in lower cabinet surfaces in the kitchen.
51 -Damaged wall covering observed in the women's toilet room. -Dust on sink and other surfaces in the staff toilet room.
52 The dumpster is lacking a lid to close tightly when not in use.
53 Observations: -Mouse droppings and debris observed on floor perimeters under shelves in the pantry area off of the kitchen. -Residue on floor perimeters in the cooking area.
53 -The mop was stored in the bucket rather than hanging on the utility rack when not in use. -The broom and dust pan were observed stored on the floor rather than hanging on a utility rack when not in use.
54 The exhaust hood in the cooking area has 2 of 3 lights not working.

14 Slicer has an accumulation of dried food residue on the inner food contact surfaces around the blade and is not being washed, rinsed and sanitized at least every 4 hours. The person in charge stated that the slicer is not in use at this time. She agreed to wash, rinse and sanitize the slicer in place to eliminate the dried food residue.
45 -The garbage grinder is not operating. -The walk in freezer is not being used at this time. There is a rack containing boxes blocking the door inside the walk in cooler. Note: There was nothing being stored in the walk in freezer. -Two other refrigeration units were observed not operating in the dining area. -Hand wash only sign observed over the 3 basin sink. Note: The 3 basin sink should be used for washing dishes only.
47 White residue observed on the basin surface of the grinder sink.
51 Damaged wall covering observed in the women's toilet room.
52 The dumpster is lacking a lid to close tightly when not in use.
54 A refrigeration unit is lacking a working light.

1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. A food safety certificate has been emailed to the person in charge.
45 The garbage grinder is inoperable.
45 Microwave oven is made with materials unapproved for commercial food equipment.
46 Food facility lacks sanitizer test strips to monitor sanitizer concentration.
51 Paper towels were not available in the faculty restroom. The person in charge provided paper towels to the restroom.
53 -Debris and old mice droppings observed on the floor behind the stove. -Dust and debris observed on the floor and perimeters of the food preparation area and dry food storage room.
53 Peeling wall paper observed in the faculty restroom is not smooth and easily cleanable.
