1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
**ServSafe was observed during the inspection. An email was sent with the application of the city of Philadelphia certification.
11 Expired Cheese observed in the reach in refrigerator in the walk-in refrigerator.
14 The following was observed:
- Black residue, pink slime on the interior of the ice machine bin around the warewash area
-The can opener had an accumulation of food residue on the food contact surfaces.
-Soda fountain dispenser with old residue
21 Cheese, mash potatoes and produce potentially hazardous ready to eat food, prepared in the food facility and held for more than 48 hours, located in the reach-n refrigerator and the walk-in units, was not date marked.
36 Visible physical evidence of flies and drain fly activity observed in the
food prep area.
45 Cutting boards observed with deep scratches and scoring not allowing for effective cleaning and sanitizing.

- 1 under counter unit used for dry storage

-Food debris around the counters and shelving units
-Food debris in reach-in units
50 Water condensation line leaking inside the walk-in refrigerator causing a pool of water inside of the walk-in unit.

53 Ceiling tiles observed peeling and are not smooth and easily cleanable.
