1 The person in charge observed with a serve safe certificate and not with valid Food Establishment Personnel Food Safety Certificate issued by the Department. * The application link was sent via email.
45 - Defective equipment( reach-in refrigerator, Pizza Under counter refrigerator and walk-in cooler ) observed in facility. - Door observed loose on chest freezer in kitchen area.
51 The following observations were made in the restroom: - Adequate lighting is not properly installed.-Portable light observed. - Cove base missing in restroom.
53 -Water stained/missing tiles in office area.
53 Mop observed stored in mop bucket and not hung to allow for proper air drying when not in use.
54 Employee personal items ( Futon bed, Boots,) Observed in kitchen area.
65 Garbage grinder observed defective.

1 The Person in Charge did not demonstrate adequate knowledge of the Philadelphia Food Code.(Food borne illnesses) * The inspector reviewed food borne illnesses with person in charge.
6 No hot water observed at hand wash sinks through out facility for employees to properly wash hands at the start of inspection. * Hot water was adjusted and observed during inspection.
8 No sign or poster posted at the hand wash sink in the restroom area to remind food employees to wash their hands. *The person in charge hand wrote temporary hand wash sign.
37 Two boxes of canola oil stored on flooring surface in kitchen area. * The person in charge properly stored on racks.
39 Moist wiping cloths observed lying on garbage grinder sink and not stored in sanitizing solution.
45 - Unused equipment( reach-in refrigerator & Under counter refrigerator) observed in facility. - Door observed loose on under counter refrigeration unit.
47 - Dust accumulation observed on fan vent covers in walk-in cooler. - Food debris observed on flooring surface of walk-in cooler under racks.
48 Hot water observed at 44 degrees F and not at the required 100 degrees F at the start of inspection. * The person in charge switched breaker on in and Hot water was observed at all hand wash sinks at 100 degrees F.
51 The following observations were made in the restroom: - Adequate lighting is not properly installed.-Portable light observed. - Cove base missing in restroom. -Restroom door is not self closing.
53 Mop observed stored in mop bucket and not hung to allow for proper air drying.
53 Brooms and dustpan observed stored on the side of walk-in cooler and not properly stored off of the ground when not in use.
53 -Water stained/missing tiles in office area. - Light switch plate cover missing in office area. - Electrical socket plate cover missing in office area.
53 Gaps observed around pipes and utility lines.
