37 Observed tiny gap between handwash sink and food prep sink, installation of splash guard advised to prevent possible contamination of food due to handwashing.
39 Moist wiping cloths observed lying on the kitchen sink and not stored in sanitizing solution.
45 The following observations were noted: -Raw wood pallets used for storage and elevation in the walk-in cooler and dry storage area. -Cardboard boxes used for storage/display.

1 Observed designated certified food safety person inaccurately preform the task of wash/rinse/sanitize unclean utensil at the warewash sink, did not demonstrate adequate knowledge of the Philadelphia Food Code. **Corrective action was taken, food employee and Health Sanitarian redirected designated food safety person on the correct wash/rinse/sanitize process to properly clean dishware and or utensils.
2 Food facility does not have a Health Awareness Reporting Procedure for food employees. PIC was not aware of all aspects of employee health restrictions and reporting requirements, including reportable illnesses.
13 The following observations were noted: -Ice scooper stored with repackaged raw meat inside the chest freezer in the food prep area. -Clean cutting board stored behind faucet head at the food prep sink. **Corrective action was taken, food employee relocated the food utensils to the warewash sink to be rewashed/rinsed/sanitized.**
26 Chemical product observed stored above the food prep sink in the food prep area. **Corrective action was taken, person in charge relocated the chemical product away from food contact surfaces/food items.**

Suitable cleaning implements and supplies (such as high-pressure pumps, hot water, steam and detergent) shall be provided as necessary for effective cleaning of receptacles and waste handling units for refuse, recyclables and returnables, except that approved off-premises-based cleaning services may be used if on-premises cleaning implements and supplies are not provided.**
53 The following observations were noted: -Raw wood boards used as floor and wall surfaces, not smooth and easily cleanable in the basement area. -Wet/water stains on raw wood board floor surfaces in the basement area. -Damaged floor tiles throughout seating and rear storage areas.

8 -There was no soap, paper towels or sign readily available in the customer toilet room. -There were no paper towels available at the hand sinks in the cooking area and the employee toilet room. All of the required items were provided during the inspection.
8 There was no hot water readily available at the customer toilet room hand sink. Hot water was turned on at the valve below during the inspection.

36 Front door opened without screening is an outer opening in the food facility that does not protect against the entry of insects, rodents, and other animals.
