**PIC took corrective action and discarded milks during time of inspection.
35 Working bulk food ingredient storage containers are not labeled with the common name of the food in food prep area.
37 Food is not stored at least 6 inches above the floor in walk-in cooler.
**PIC took corrective action and elevated food off of floor during time of inspection.
41 Several food dispensing utensils in the bulk ingredient containers and ice bin were observed stored in direct contact with food and not having handle stored above the top of the food.
45 Milk crates observed being used for elevation of food in walk-in freezer which are not smooth and easily cleanable.

1 A Food Safety Certified person was not present at the start of inspection.
**No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
**Food Safety Certified person arrived by end of inspection.
8 Paper towel dispenser empty at the handwashing sink in the unisex employee restroom area.
**PIC took corrective action and provided paper towel at handwashing sink during time of inspection.
34 Temperature measuring devices for ensuring proper temperatures of food are not available or readily accessible in under-the-counter refrigeration unit in food prep area.
36 The following were observed:
-Visible physical evidence of rodent activity (mouse feces) observed around the perimeters of floor in rear dry storage area and under front service counter in food prep area.
-Visible physical evidence of insect activity (dead flies) observed on glue trap in food prep area.
**Dead or trapped birds, insects, rodents and other pests shall be removed from control devices and the premises at a frequency that prevents their accumulation, decomposition or the attraction of pests.
-An insect control device (glue trap) was located on ceiling in food prep area directly above food prep table with potential to contaminate food, equipment, and / or utensils.
37 Food is not stored at least 6 inches above the floor in walk-in cooler.
**PIC took corrective action and elevated food off the floor during time of inspection.
45 The following were observed:
-Damaged interior gasket on under-the-counter refrigeration unit in food prep area
-Broken hand dryer in unisex employee restroom area
45 Food contact surface of cutting board in food prep area is not smooth, easily cleanable and resistant to pitting, cracking, or scratching.
**PIC was instructed to replace cutting board during time of inspection.
45 Milk crates observed being used for elevation of food in walk-in freezer which are not smooth and easily cleanable.

-Food debris/residue around perimeters of floor in walk-in cooler/freezer
-Static dust accumulation on shelving units in food prep area/walk-ins
