1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. City of Philadelphia Food Safety Certificate Application will be emailed with inspection report.

11 Observed (1) container of yogurt open inside open display case in retail area. -PIC discarded immediately

14 Indirect drain observed installed above 1 out of 3 basins of sanitization sink. Orders: Remove plumbing fixtures immediately

14 Food safety person did not properly demonstrate wash, rinse, and sanitize procedures when asked by Sanitarian at time of inspection. Labeling of sanitization procedures were also observed incorrect. *Sanitarian explained proper sanitization procedures at time of inspection

26 Sanitizers. Chemical sanitizers and other chemical antimicrobials applied to food-contact surfaces shall meet the requirements specified in 21 CFR 178.1010 (relating to sanitizing solutions). -Unapproved sanitizer observed in use at time of inspection. PIC obtained approved sanitizer at time of inspection

41 Serving utensils located inside ice bin (scoop with handle) stored with food contact surfaces exposed to hand contact when retrieved for service.
