1 Food Safety Certified person did not demonstrated knowledge of five Food Borne Illnesses. **PIC was taught the five Food Borne Illnesses, their signs and symptoms, and issued the Food Employee Health Policy during time of inspection.
13 - The food was not covered in the walk-in cooler and reach-in refrigerator during inspection.
14 The chlorine concentration in the sanitizing solution of the 3-bay warewash sink was more than 200 ppm, rather than 50-100 ppm as required. PIC took corrective action and provided sanitizing solution at 100 ppm during inspection.
21 -Potentially hazardous ready to eat food (cooked meats, vegetables, and sauces) prepared in the food facility and held for more than 48 hours in the walk-in coolers and reach-in refrigerators. *Sanitarian advised the person in charge to provide dates for the foo in the facility.
26 Rodenticides observed on the floor near the hot water heater in the basement area.

36 The following observations were noted: - An insect control device (sticky glue paper) with dead bugs on, located in top of the cold prep unit in the prep area with potential to contaminate food, equipment, and / or utensils. - Visible physical evidence of bugs activity (live bugs) observed near the water heater in the basement area.

14 - The can opener observed with heavy accumulation of food residue on the food contact surfaces. - The warewash sink was not properly setup for sanitation; dirty dishes and pots observed stored in all three 3 bay sinks. Corrective action: Sanitarian educated the PIC on how to properly setup the 3 comparment sink for sanitation. A return demonstration of Wash-Rinse-Sanitize setup was observed prior to completion of inspection.
