8 Paper towels were not observed at the kitchen's hand wash sink. *Person in charge brought paper towels, correction on site.
13 The following observations were noted: -Single-use serving container without handle submerged in food item stored in one container in the food prep area. *Person in charge added correct scoops and corrected on site.
21 Opened commercially processed ready to eat food, located in the walk in cooler, and held more than 48 hours, was not marked with the date it was opened. *Person in charge corrected and added date markings.
35 The food in the bulk containers were not labeled with their common use names.
37 Case of oil stored on direct floor surfaces, is not stored at least 6 inches above the floor.

22 Food was kept outside for use without any time stamps to track the 4 hours. Note: The person in charge placed the chicken and egg rolls in the refrigerator.

21 No dates for food items throughout the facility (reach-in refrigerators, under-counter refrigerators, and walk-in cooler). *Sanitarian advice the person in charge to provide dates for all the food items
36 Visible physical evidence of rodent (mouse droppings) observed on the floor and on the bottom shelves of the prep tables throughout the facility. Insect activity (house flies) observed in the dry storage area.
37 - Food containers (soy sauce and meats) observed not stored at least 6 inches above the floor in the walk-in freezer and cooking area.
45 - One NON-NSF/ANSI microwave "Panasonic" observed in the kitchen area. - Domestic "GE" freezer observed in the prep area.

8 The handwash sink near the dinning area was in need of cleaning. A food employee cleaned the sink during inspection.

14 The can opener had an accumulation of food residue on the food contact surfaces.
14 The chlorine concentration in a 3-bay sink was greater than 200 ppm, rather than 50-100 ppm as required. PIC initiated corrective action during inspection.
