14 The chlorine concentration in the sanitizing solution of the 3-bay warewash sink was 10 ppm, rather than 50-100 ppm as required. The person in-charge added one more chlorine and it was measured 50ppm.
45 Observations: -Garbage grinder not operational. -One of the walk-in freezer fans was not functioning. -No operational light for one of the reach-in refrigerators. -Ice cream freezer not in use. -Chest freezer not in use. -A leak observed beneath 3 basin sink.

1 The person in charge did not know the names of the five foodborne illnesses required by the Department. *By the end of the inspection, the person in charge was able to name the five foodborne illness-causing organisms.
8 Observations: -No sign was observed to remind food employees to wash their hands at the handwashing sink in the storage toilet room. *The person in charge put a sign to remind food employees to wash their hands during the inspection. -Soap was not available at the handwashing sink in the storage toilet room, men's toilet room, and cool food prep area. *The person in charge provided soap during the inspection. -The paper towel dispenser was empty at the handwashing sink in the storage toilet room, women's toilet room, men's toilet room, and cool food prep area. *The person in charge provided paper towels during the inspection. -The temperature of the hot water at the handwashing sink in the food prep area was measured at 81 degrees F at the beginning of the inspection. *The hot water was measured at 100 degrees F by the end of inspection.

1 The Person in Charge did not demonstrate adequate knowledge of the Philadelphia Food Code. *The person in charge could not demonstrate proper sanitizing techniques for washing, rinsing and sanitizing utensils and cooking equipment at the 3 basin sink. *The Sanitarian reviewed the proper washing, rinsing and sanitizing method with the person in charge and the PIC was able to demonstrate it.

38 Food employees observed not wearing or wearing ineffective hair restraints, such as beard restraints. *The person in charge provided nets for his employees. *Employees shall wear hair restraints such as hats, hair coverings or nets, beard restraints and clothing that covers body hair, that are effectively designed and worn to keep their hair from contacting exposed food; clean equipment, and utensils.
43 Utensils and bowls observed not inverted in the kitchen area. *Clean equipment and utensils shall be stored covered or inverted, and in a self-draining position that allows air drying.
45 Observations: - Non NSF approved cooler (iced bowl) used to keep beverages cold in the Main Kitchen. - Cold prep refrigerator and refrigerator are out of order in the nutrition room. - Cold prep refrigerator is out of order in the stock room. - Repair air conditioner (AC unit dripping water) in the kitchen area.

8 The air conditioning unit condenser line was observed draining into the handwashing sink in the Nourishment room. A handwashing facility may not be used for purposes other than handwashing. -The person in charge rerouted the condensate line to drain into a bucket.

14 The quarternary ammonia concentration in the sanitizing solution of the 3-bay warewash sink was 500 ppm, rather than 200pm as stated on the manufacturer's use directions. The person in charge added water to bring the concentration to the appropriate level.

Note: The person in charge was instructed to immediately wash, rinse, and sanitize the prep tables and clean lid that had mice feces and dead insect. Corrective action was taken.

The hot water final rinse reached 110 degrees F instead of 180 degrees F as listed on the machine's data plate. **Note: The final hot water temperature could not be verified during the inspection. The sanitarian advised the person in charge to use the three bay sinks for washing, rinsing, and sanitizing until the final temperature could be verified.
