1 The Person in Charge did not demonstrate adequate knowledge of the Philadelphia Food Code. *The person in charge could not demonstrate proper sanitizing techniques for washing, rinsing and sanitizing utensils and cooking equipment at the 3 basin sink. *The Sanitarian reviewed the proper washing, rinsing and sanitizing method with the person in charge and the PIC was able to demonstrate it.
35 Flour storage container was not labeled with the common name of the food.
38 Food employees observed not wearing or wearing ineffective hair restraints, such as nets, hats, or beard restraints. *Employees shall wear hair restraints such as hats, hair coverings or nets, beard restraints and clothing that covers body hair, that are effectively designed and worn to keep their hair from contacting exposed food; clean equipment and utensils.
41 Common bowls or food containers observed without handles in direct contact with the flour. *Food dispensing utensils shall be stored with their handles above the top of the food within the containers.
43 Salmon cakes and other foods in the chest freezer observed stored in "Strohman Bread" bags. Single-service and single-use articles may not be reused.
45 -Non NSF approve chest freezer observed in kitchen. -Soda crates used for elevation and plastic storage food containers (walk in cooler) are not made of food grade materials.
47 Accumulation of grease observed on and beneath the cooking equipment. *The food-contact surfaces of cooking equipment and pans shall be kept free of encrusted grease deposits and other soil accumulations.
49 Observations: -Garbage grinder inoperable -Leakage beneath the hand washing sink.
53 Broom, mops, and dust pan observed being stored in mop sink and not hung properly for storage. *Maintenance tools such as brooms, dust pan, mops, vacuum cleaners and similar items) shall be stored so that they do not contaminate food, equipment and utensils.

54 Observed accumulation of grease and dust hood filters. *Ventilation hood systems and filters. Filters or other grease extracting equipment, if not designed to be cleaned in place, shall be designed to be readily removable for cleaning and replacement.

8 No sign or poster posted at the hand wash sink in the women's restroom to remind food employees to wash their hands.
13 Foods in the under counter refrigerators, chest freezers, and walk in cooler observed uncovered from contamination.
14 An employee's cellphone observed placed on the rear cutting board by the flat top grill.
21 Cheese opened commercially processed ready to eat food and sliced tomatoes, green peppers, and cooked potatoes, located in the under counter refrigerator and walk in coolers, was not marked with the date it was opened.
35 Flour storage container was not labeled with the common name of the food.
36 Visible physical evidence of rodent, mice feces, observed on the can tops of the corn beef hash in the rear of the food prep area, underneath the grill and flat top grill, floors behind the cooking equipment, and next to the chest freezer in the front food prep area.
38 Food employees observed not wearing or wearing ineffective hair restraints, such as nets, hats, or beard restraints.
