8 Paper towel dispenser empty at the hand wash sink in the kitchen and restroom area.
8 Soap was not available at the hand wash sink in the kitchen area.
* Soap was provided.
14 The quaternary ammonia concentration in the sanitizing solution of the 3-bay ware wash sink was 400 ppm, rather than 200-300 ppm as stated on the manufacturer's use directions.
* The person in charge added correct amount of water. Sanitizer observed at 300ppm.
26 Chemicals were stored above or on the same shelf with food (bottled sauces), equipment, and/or single service articles in the ware wash area.
*Bottled sauces were relocated to food prep area.
36 -Visible physical evidence of insect activity (2 German roaches) observed in rear storage area of kitchen behind refrigeration unit.
** Sanitarian advised that a professional pest service be obtained prior to requesting a re- inspection. Also provide receipt of service provided.
37 Cases of soda observed stored on hallway floor and not elevated off of the ground.

- Heavy grease accumulation on floor surfaces of hot hold unit.
- Debris and dust along floor perimeter under 3 compartment sink.
- Food debris on wall surfaces of rear storage area.

-Doors not cleaned to sight in establishment.
*General cleaning is needed.
53 Broom and dustpan observed not elevated off of the ground when not in use.
