8 No soap or dispenser at the bar hand sink. The dispenser was placed in the area but it is not attached to a surface.
13 Containers of foods (olives, cherries and beverage ingredients) stored uncovered in the box refrigerators and small refrigerators in the bar area. Covers were provided.
20 Hamburgers, pork, and shrimp were held at temperatures above 41 degrees F rather than 41 degrees F or below as required. Since these items were stored in this unit for four hours or more they were discarded.
21 Drink ingredients were not date marked in the box refrigerator. Dates were placed on these containers.
38 Food employees observed not wearing or wearing ineffective hair restraints (beard restraints).
45 Splashguard lacking between the garbage grinder sink and the food prep sink. Note: The person in charge stated that the prep sink is not used. All produce ingredients come prepackaged.
45 The garbage grinder sink in the kitchen was lacking water. The water was turned on at the valve below.
47 -Residue observed on refrigerator inner surfaces in the refrigeration units. -Residue on floor perimeters in the walk-in unit.
53 -Debris observed on the floor perimeters in the kitchen area.
54 A designated place for employees to store personal items is lacking. Employee belongs observed stored in equipment and in grinder sink in the bar and kitchen area.
63 Person in Charge of premises has not posted the appropriate warning signs regarding the dangers of consuming alcoholic beverages during pregnancy.

36 Visible physical evidence of insect (small flies) activity observed in the bar area.
47 -Small flies observed on surfaces in the keg refrigerator. -Residue observed on refrigerator inner surfaces in the refrigeration units. -Residue observed on shelving in the cabinets.
53 -Debris and residue observed on the floor perimeters under equipment.
53 The dust pan was observed on the floor rather than hanging on a utility rack.
