13 Raw chicken were stored above ready to eat foods in the cold hold unit. Person in Charge took corrective action and relocated chicken.
14 Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was 10 ppm, and not 50-100 ppm as required. Person in Charge took corrective action and provided a fresh bottle of sanitizer.
36 Damaged cellar doors, outer opening in the food facility does not protect against the entry of insects, rodents, and other animals.
37 Food is not stored at least 6 inches above the floor in storage areas.
41 - Scoops for rice were observed being held in a container of stagnant water between uses.

14 Pink slime observed on the interior white panel of the ice machine bin. Cleaning procedures were initiated during the inspection.
35 Working bulk food ingredient storage containers are not labeled with the common name of the food.
36 A glue trap with dead cockroaches was observed in the basement area. The glue trap was removed during the inspection.
39 Moist wiping cloths observed lying on prep surfaces and not stored in sanitizing solution.
41 - Food dispensing utensil in the ice machine observed in direct contact with food and not having handle stored above the top of the food. - Scoops for rice were observed being held in a container of stagnant water between uses.
43 Takeout containers were not stored inverted to prevent contamination.

45 Cutting boards observed with deep scratches and scoring not allowing for effective cleaning and sanitizing.

13 -Several raw animal foods were stored above ready to eat foods in the cold hold unit and reach-in refrigerator. Person in Charge took corrective action and relocated items. -Uncovered peanuts and sauces observed in food preparation area. Person in Charge took corrective action and covered items.
14 Black residue, pink slime observed on the interior of the ice machine bin. Person in Charge burned ice and washed, rinsed and sanitized surfaces.
26 Working containers in server station area, used for storing chemicals taken from bulk supplies, were not marked with the common name of the chemical. Person in Charge took corrective action and labeled bottle.
34 Temperature measuring devices for ensuring proper temperatures of equipment are not available or readily accessible in refrigeration equipment.
