39 Moist wiping cloths observed lying on kitchen surface area and not stored in sanitizing solution.
45 Cutting board observed with deep scratches and scoring not allowing for effective cleaning and sanitizing. *Person in charge disposed of cutting board.
47 The following observations were noted: - Food debris (flour) observed on the equipment in the bisques area. - Food debris observed in several under-counter refrigerators and reach-in refrigerators prep area. -Static dust on fan cover observed inside of one of the walk-in cooler units.
53 Debris observed on the floor in food prep area, coffee area, and ware washing area.

14 -The quaternary ammonia concentration in the sanitizing solution of the 3 bay warewash sink was greater than 500 ppm, rather than 200-400 ppm as stated on the manufacturer’s use direction *PIC initiated corrective action and lowered concentration to 200 ppm.
45 One reach-in refrigeration unit located in the food prep area observed not in use.
53 - Food debris observed on the floor in food prep area. - Clutter observed in the warewashing area and storage area (near the ice machine).
53 Brooms and dustpans observed stored on floor and not elevated on a utility rack in designated areas.

8 The following were observed: -The handwashing sink in the rear food prep area was blocked by a broom and dustpan and not accessible at all times for employee use -The handwashing sink in the front food prep area was blocked by cleaning machine and not accessible at all times for employee use -Old food debris/residue observed in the handwashing sink in the drive-thru food prep area **PIC took corrective action and removed equipment from blocking handwashing sink and properly washed, rinsed, and sanitized interior surfaces of handwashing sink in drive-thru area.
11 Potentially hazardous food was held at 118-128 degrees F rather than 135 degrees F or above as required. **PIC was instructed to discard potentially hazardous food during time of inspection.

26 Chemicals were stored on the under-the-counter refrigeration unit in food prep area and on the same shelf with food equipment (clean dishware) in the rear dry storage area near 3 bay ware-washing sink. **PIC took corrective action and moved chemical bottles to a chemical only designated area during time of inspection.
