38 Food employees observed not wearing or wearing ineffective hair restraints, such as nets, hats, or beard restraints. Employees put on the required hair restraints.

45 Equipment not in good repair. -A part is hanging in the ice machine. An employee began working to fix the part. The inspector requested that the ice be emptied since the employee was working over the ice storage area. -A refrigerator is not in use in the prep area.

47 -Observed oil on surfaces in the cooking area.

49 A backflow prevention device was not observed on the water filter inlet pipes.

52 Observed some oil residue on the ground in front of the used cooking oil dumpster.

53 Observations: -Unused refrigerator in the prep area. -Oil and residue on floor perimeters under the cooking equipment. -Water and general debris under equipment in the drive thru area. -Beverage residue under the trash cans in the dining area.

53 Observations: -Peeling paint on the office doorway off of the prep area. -Flashing on the walk-in unit floor is loose and can be a tripping hazard.

54 The light in the women's toilet room is dim.
