39 Moist wiping cloths observed lying on kitchen surface area and not stored in sanitizing solution.
45 Aluminum foils wrapped over stove area.
45 -Domestic Non NSF/ANSI chest freezer observed in the basement -Milk crates were utilized to elevate items in kitchen area.
45 Cutting boards observed with deep scratches and scoring not allowing for effective cleaning and sanitizing.
47 Observed grease and dust accumulation on some filters under the rear kitchen exhaust hood system.
47 - Food debris observed in some reach-in refrigerators basement area. - Debris and grime present on floor inside walk-in coolers and walk-in freezer.
49 No vacuum breaker or backflow prevention device installed on utility (mop) sink's faucet.
51 - Observed dust on ceiling vent in the toilet room. - Water stains observed on ceiling tiles employee's restroom.
53 - Dust, dirt, and food debris observed behind the food equipment in the kitchen.

14 The following observations were noted: - The ware wash ( 3-compartment ) sink not clean to sight and not properly setup for sanitization of multi-use utensils and food equipment, debris present in all bays of the 3-compartment sink. Corrective action: Sanitarian explained to the person in charge how to properly setup the 3 -comp sink for sanitation of utensils and food equipment. A return demonstration of Wash-Rinse-Sanitize setup was observed prior to completion of inspection. - The can opener had an accumulation of food residue on the food contact surfaces. The can opener was washed, rinsed, and sanitized during inspection.
21 - Several food items observed without dates in the reach-in refrigerator from basement area and under-counter refrigerators. PIC provided dates for all food during inspection.
37 Boxes with oil and beverages observed on the floor and not elevated at least 6 inches above the floor in the facility.
45 - Aluminum foils wrapped over bottom shelves of prep tables basement area.
46 - Food facility lacks chlorine sanitizer test strips to monitor sanitizer concentration.
47 Food debris observed in some reach-in refrigerators basement area.
51 No lid observed on trash can in employee's restroom basement area.
51 - Missing ceiling tiles in the employee's restroom. - Water stains observed on ceiling tiles employee's restroom.
53 The following observations were noted: - Peeling paint observed on the floor in the basement area. - Porous ceiling tiles observed in the first kitchen area.
53 The following observations were noted: -Water damaged ceiling surfaces were observed throughout the facility. - Missing ceiling tiles in the basement area.
54 - Insufficient light observed in the walk-in cooler/basement area.

1 The Person in Charge did not demonstrate adequate knowledge of the PA Food Code. The Food Safety certified person has no adequate knowledge of the PA Food Code. The PIC could not identify the 5 food borne illnesses that need to be reported to the Health Dept. Sanitarian explain to the certified person at the time of inspection.
14 Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was 0 ppm, and not 50-100 ppm as required: PIC took unit out of service and will will utilize alternate 3-bay sink for cleaning, washing, and sanitizing (basement area).
21 Several cooked food observed without dates during inspection. PIC took corrective action and provided dated immediately.
37 Boxes with oil, bags with flour, and beverages observed on the floor and not elevated at least 6 inches above the floor.
38 Food employees observed not wearing hair restraints, such as nets.
45 The following observations were noted: -Domestic microwave and residential refrigerator/freezers were observed in the facility. -Plastic light cover observed cracked in the prep area.
46 Food facility lacks the quaternary ammonia sanitizer test strips to monitor sanitizer concentration.
47 Food debris observed in some reach-in freezers from the basement area.
51 Ventilation system observed with dust accumulation in both restrooms.
53 The following observations were noted: -Porous ceiling tiles observed in the food prep areas which are not smooth and easily cleanable. -Chipping paint floor surfaces were observed in the basement. -Water damaged ceiling surfaces were observed throughout the facility. -Cracked/damaged flooring tiles observed in the entryway of the kitchen.
