42 Plates were not properly inverted in the food preparation area.
45 The following observations were noted:
-Unused equipment in the basement storage area and warewashing area.
-The faucet of the food preparation sink located in the basement area was dripping water when turned on.
-The interior gaskets of several refrigeration units were in need of repair.
-A non NSF/ANNSI fridge located in the basement storage area.
45 The following observations were noted:
-Cardboard used to line the interior of one under counter refrigerator located in the food preparation area.
-Plastic crates used for elevation in the basement walk-in cooler.
47 The following observations were noted:
-The floors of the walk-in coolers were in need of cleaning due to debris and residue.
-Ice build-up in several reach-in freezers located in the basement storage area and pizza station area.
-The interior lower surfaces of several refrigeration units were in need of cleaning due to residue.
-Rust around shelves in multiple refrigeration units.
48 The following observations were noted:
-Low water pressure at the handwashing sink located in the pizza station area.
-A hot water delay at the handwashing sinks located in the pizza station area and restroom areas. Hot water took several minutes to reach peak demand.
51 The handwashing sink faucet located in one of two restrooms was loose and in need of tightening.
53 The following observations were noted:
-Floor cleaning was needed in under and around equipment in the food preparation area due to debris.
-Floor cleaning was needed in the bar area due to debris.
-Floor cleaning was needed under the warewashing equipment located in the basement due to food debris and residue build-up.
-Floor cleaning needed under shelving units in the rear basement storage area due to debris and old mouse droppings.
53 The following observations were noted:
-Missing ceiling tiles in the food preparation area.
-Water-stained ceiling tiles in the food preparation area.

11 Expired milk was observed in the walk-in cooler dated, 7/16/2023. The person in charge discarded the expired milk.

-The floors of the walk-in coolers were in need of cleaning due to debris and residue.
-Ice build-up in several reach-in freezers.
51 The floors in one of two restrooms were in need of cleaning due to debris.
53 Floor cleaning was needed in the basement warewashing area and storage areas due to debris, residue, and stagnant water.
53 Damaged floor tiles were observed in several areas.

1 The person in charge only has a ServSafe certificate. An application was re-issued to the person in charge via email.
8 The handwashing sink located in the basement food preparation area was blocked by multiple containers and items and was not accessible. The person in charge was informed that the handwashing sink must be accessible at all times.
14 Pink slime was observed on the interior of the ice machine bin around the upper rim. The person in charge was informed to discontinue the use of the ice machine until it is properly cleaned and sanitized. The person in charge unplugged the ice machine and begun the process to scoop/discard the ice in the machine.
21 Opened commercially processed ready to eat food (various cheeses), located in the basement walk-in cooler, and held more than 48 hours, was not marked with the date it was opened.
36 The following observations were noted:
-Mouse droppings on the floor in the bar area.
-Fruit flies in the basement warewashing/food preparation area.
42 Plates and bowls were not properly inverted in the food preparation area.
45 Cardboard was being used to line interior surfaces of equipment in several refrigeration units.
45 Unused equipment was observed in multiple areas of the establishment.
47 The following observations were noted:
-Exterior/interior surface cleaning of equipment due to debris and residue.
-Ice build-up in several reach-in freezers.
53 Damaged floor tiles were observed in several areas.

-Floor cleaning was needed under and around equipment in the food preparation areas due to debris and residue.
-Floor cleaning was needed under equipment in the bar area due to debris and mouse droppings.
-Floor cleaning was needed in the warewashing areas due to debris, residue, and stagnant water.
-Drains located in the basement area were in need of cleaning due to old food debris.
53 Brooms and dust pans were not properly elevated off of the floor in several areas.
