1 The food facility operator shall be the Person in Charge or shall designate a Person in Charge and shall ensure that a Person in Charge is present at the food facility during all hours of operation.
1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. *Application for City of Philadelphia Food Safety Certificate will be emailed with inspection report.
8 The handwash sink in the front serving and near warewash sink areas were blocked by food related equipment and not accesible at all times for employee use.
9 Prepackaged juice shall be obtained from a processor with an HACCP system as specified in 21 CFR 120 (relating to hazard analysis and critical control point HACCP systems) and be one of the following: (1) Obtained pasteurized or otherwise treated to attain a 5-log reduction (a 99.999% reduction) of the most resistant microorganism of public health significance as specified in 21 CFR 120.24 (relating to process controls). (2) Bear a warning label as specified in 21 CFR 101.17(g) (relating to food labeling warning, notice, and safe handling statements). *observed bottled juice containing tamarind fruit without labeling reported by person in charge to be prepared within facility.
14 Person in charge could not properly demonstrate wash, rinse and sanitizing procedures at time of inspection.
20 Foods stored in food prep unit was held at 50-52 degrees F rather than 41 degrees F or below as required. *Orders: discard all potential hazardous foods stored in defective food prep unit immediately and discontinue usage until repaired *Food items included cut lettuce, sauces, rice and chicken
27 Food facility is preparing and packaging "Tamerind Fruit" juice without a HACCP plan or any means of providing for a 5 log reduction of pathogens, or alternatively providing a consumer advisory or warning label on the juice. *observed bottled juice containing tamarind fruit without labeling reported by person in charge to be prepared within facility.
37 Cases of water bottles observed on flooring in front serving area.
45 Observed defective food prep unit maintaining improper temperatures of 50-52 degrees F. Discontinue usage of defective unit until repaired.
46 Food facility lacks sanitizer test strips to monitor sanitizer concentration.
47 Chest freezers observed with heavy ice accumulation.
53 Perimeter of flooring in basement observed in need of cleaning.
54 Observed missing light on hood unit (1 out of 3).
