8 -Handwashing sink in kitchen was measured at 98 degrees F. -Toilet room handwashing sink was measured at 96 degrees F. *Corrective Action for toilet room: A technician increased hot water heater. Toilet room is now 100 degrees F. **Kitchen is currently closed.
13 The rim above the ice within the ice machine was observed with black residue accumulation.
43 Single use containers / take home containers were not inverted while stored on table.
46 The wash/water temperature of the three compartment sink measured at 96 degrees F.
47 -Slight grease residue accumulation observed on the sides of fryer in the kitchen. -Polyfilm adhesive was observed on both the reach in freezer, refrigeration unit, and newly installed prep table. This surface is not smooth/easily cleanable.
49 No designated utility sink was observed for the proper disposal of mop/waste water.
51 A separate trash receptacle for the disposal of women's sanitary products was not available in the bar restroom.
53 -Dirt residues observed on floor perimeter under handwashing and food prep sink in the kitchen area.
54 The toilet room vent in the women's restroom was observed covered up by a sign and cannot fully function.

1 The Person in Charge shall ensure that employees are properly trained in food safety as it relates to their assigned duties. Observation: a new employee working in the kitchen did not have knowledge about the proper sequence in ware washing (wash, rinse, and sanitize) and cold food temperatures. These topics have been discussed with the Person in charge & employee.
8 The hand wash sink at the bar and kitchen areas did not have single use towels, continuous towels, or air drying device. Corrective Action: Disposable paper towels were placed.
8 Hot water was not observed at the bar hand wash sink. Corrective Action: The water was fixed during the inspection.
13 Drink ice at the bar was observed with beer bottles within. Ice is being used for both chilling and within drinks which does not protect from contamination and is unapproved.

14 The three basin sink in the kitchen was observed improperly set up, the employee displayed a lack of knowledge, and there was no sanitizer included. Corrective Action: The proper sequence of wash, rinse, and sanitize was discussed. The employee properly set the sinks up including sanitizer in the last basin.
20 Cooked chicken, beef, and salmon was held at 47 degrees F rather than 41 degrees F or below as required. These items were cooked the previous night. Corrective Action: The proper cold holding temperatures were discussed with employee and PIC. All items noted above were discarded.
34 Temperature measuring device for ensuring proper temperatures of food are not available or readily accessible; Probe thermometer.
36 Visible physical evidence of rodent activity observed in the kitchen area; mouse droppings.

46 Designated drainboard/ air drying rack above three basin sink was observed being used for seasonings storage. Food equipment/containers observed cluttered on the side of the three basin sink instead. -Seasonings were removed. Once dishes are cleaned they will be properly stored onto drain board.
47 -Heavy grease accumulation observed on the sides of cooking equipment in kitchen. -Grease accumulation observed on mechanical hood and filters in kitchen. -Mouse droppings observed on / in domestic cabinet surface near microwave in kitchen. -Interior of reach in refrigeration unit in kitchen was observed with residue accumulation at the bottom. -The exterior of the hand wash sink and knobs within kitchen observed with residue accumulation. -Polyfilm adhesive was observed on both the reach in freezer and refrigeration unit. This surface is not smooth/easily cleanable. Residues and piling of the adhesive observed.
49 No designated utility sink was observed for the proper disposal of mop/waste water.
51 A separate trash receptacle for the disposal of women's sanitary products was not available in the bar restroom.
53 -Open utility penetration observed under the par hand wash sink. The opening around the pipe is not sealed. -Small holes observed in the kitchen wall by back door and on the side of the reach in freezer.
53 -A few water stained ceiling tiles were observed in the bar area. -Heavy residue accumulation observed in between / under cooking equipment in the kitchen. -Mouse droppings observed along some of the floor perimeters in the kitchen. -Residue accumulation observed on indirect drains at bar ice machine and under prep sink in the kitchen. -Soiled cement block were observed next to cooking equipment in kitchen.
54 Burned out light bulb observed under mechanical hood and within walk-in cooler.
