1 The food facility operator shall be the Person in Charge or shall designate a Person in Charge and shall ensure that a Person in Charge is present at the food facility during all hours of operation. *PIC arrived by the end of the inspection*
6 Handwash sink in the employee toilet room was observed with no running water. Food employees shall clean their hands and exposed portions of their arms after using the toilet room.
*Employees were instructed to use the employee handwash sink located in the front prep area outside of the restroom until the handwash sink in the toilet room is properly serviced*
8 The handwash sink in the front prep area was blocked by a garbage can and not accessible at all times for employee use. *garbage can was relocated at the time of inspection*
13 Raw eggs observed being stored over other raw, ready-to-eat food items in the under-counter refrigeration unit in the rear prep area.
20 multiple food items (several types of cream cheeses) were held at 49-57 degrees F rather than 41 degrees F or below as required. *These items were discarded at the time of inspection*
36 Visible physical evidence of insect activity observed: one live roach observed in the front prep area; dead roaches observed in the basement.

-observed two defective open display cases at the front retail area not in use at the time of inspection; both units also missing kickplates.
-broken lid observed for cold prep unit to the right of the toaster which is causing top cold prep unit to not hold proper temperatures- cream cheeses stored here were discarded.

Surfaces of equipment that are nonfood-contact surfaces, but are exposed to splash, spillage or other food soiling or that require frequent cleaning, shall be constructed of a corrosion-resistant, nonabsorbent and smooth material.

4 Food employees were observed drinking coffees in the front prep/ service area in between assisting customers. *Coffees were discarded at the time of inspection*

26 quaternary sanitizer solution in the sanitizer bucket located at front prep area- was observed being used in excess of 500 ppm on food contact surfaces. *PIC diluted the sanitizer to proper PPM at the time of inspection*
36 Visible physical evidence of insect activity observed: one live roach observed in utility room and heavy accumulation of dead roaches observed throughout the basement prep area/ basement storage area on glue traps.
*Removing dead insects: dead insects shall be removed from control devices and the premises at a frequency that prevents their accumulation, decomposition or the attraction of pests*

1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
**Sanitarian emailed the person in charge a copy of the Philadelphia Food Safety Certificate Application.
