45 Equipment does not meet minimum design standards. -Non-commercial Kenmore chest freezer. -Panels missing on the soft serve ice cream machine.
46 Sanitizer test strips not stored in the original container with proper labeling and legend to monitor sanitizer concentration.
47 Observations: -Rust observed in the freezers.
51 Shared toilet rooms observations: -The men's toilet room soap dispenser is not wheel chair accessible. It is located high on the wall over the sinks. -Both urinals are covered in the men's toilet room. -The handicap stall is lacking a door in the women's toilet room. -A faucet assembly is loose at the hand sink in the women's toilet room. -Water stained ceiling tiles
53 Observations: -Unused equipment observed in the establishment. For example, frozen dessert machine, popcorn dispenser, tabletop heated display case, and a pretzel hot holding unit.
53 -Baseboard coving on the wall and floor junctures are not sealed. -Stained and defective ceiling tiles observed. -Dark colored surfaces (floor tiles) make inspection and cleaning difficult.
54 Dim overhead light in the warewash area.

8 The water was turned off at the hand sink in the prep/cooking/warewash area. The person in charge turned on the hot water during the inspection.
8 Soap was not available at the handwash sink in the prep/cooking/warewash area. The person in charge provided soap.
13 Food not protected from contamination in refrigerators. -Hot sausages, and animal food, was stored in containers above prepared foods and beverages in the refrigerator. -Pie shells not covered in the large refrigerator.
14 Not able to wash, rinse, and sanitize surfaces properly. Stoppers were not readily available for the 3 basin sink.
21 Pie fillings (sweet potato pie filling), potentially hazardous ready to eat food, prepared in the food facility and held for more than 48 hours, located in the refrigerator, was not date marked.
22 Time in lieu of temperature being used in the food facility to control ready to eat potentially hazardous foods without written procedures or documentation to verify disposition of food. Prepared sweet potato pies and/or pudding observed displayed on the table were not kept at 41 degrees F or lower. There was no time/temperature log observed. All potentially hazardous foods held out of temperature for four hours or more must be discarded.
35 Prepackaged food (pies, cakes, cobblers and cookies on display table) is not labeled properly with the name of product, ingredient statement, net weight, distributed by statement, dates, and/or nutritional facts.
36 Visible physical evidence of rodent (mouse droppings)/ insect (live roaches) activity observed throughout the facility.
45 -The hand sink was lacking cold water.
47 Observations: -The hand sink had food residue in the basin. -The 3 basin sink is dirty. -Live roach and mouse droppings observed on the pie press. -Mouse droppings observed on items and shelves in the warewash area. -Accumulated debris in the gaskets of the water ice freezer. -Rust observed in the freezers. -Dust and debris on the outer surfaces of equipment throughout.
51 A lidded can was lacking in the shared women's toilet room. A lid was placed on the trash can.
51 -Both urinals are covered in the shared men's toilet room. -Water stained ceiling tiles in the shared toilet rooms.
53 Observations: -Mouse droppings, live roaches, dust and debris on floor perimeters under and behind equipment. -Unused equipment, excess items and clutter observed in the establishment. For example, frozen dessert machine.
53 -Baseboard coving on the wall and floor junctures are not sealed. -Stained and defective ceiling tiles observed.
54 -Light bulb in the rear warewashing area is not shielded from breakage. -Lights in the front prep/cooking area are not shielded from breakage.
