1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.

1 The food facility operator shall be the Person in Charge or shall designate a Person in Charge and shall ensure that a Person in Charge is present at the food facility during all hours of operation.

6 A food employee was observed washing their hands at the prep sink rather than the designated handwash sink.
8 Observed the following:
-No sign or poster posted at the handwash sink in the food prep area to remind food employees to wash their hands.
-Bucket and bottle inside handwashing sink.
13 Observed the following:
-Raw meat products (chicken, pork, beef) stored above sauces in walk-in cooler.
-Uncovered food (onions, tomatoes, peppers) inside walk-in cooler
-Uncovered food (cheese) inside under counter refrigerators

13 Observed condensation from walk-in freezer draining into food prep sink.
36 Visible physical evidence of rodent (mice) activity observed throughout facility.
36 An insect control device (fly paper) located in food prep area with potential to contaminate food, equipment, and / or utensils.
37 Food (Ice used for drinks) is not stored at least 6 inches above the floor in walk-in freezer.
41 Common bowls or food containers observed without handles in direct contact with bulk food ingredients (spices).

53 Observed porous/water stained ceiling tiles in food prep area that do not allow easy cleaning.
53 Observed broom, dustpan, and mops stored on floor rather than on a utility rack.
53 Cleaning needed in the following areas:
-Perimeters throughout facility due to mouse droppings
-Mop sink in rear storage area
-Ceiling vents in food prep area
54 Observed the following:
-Employee personal items (bags) in rear food storage areas.
-Employee phone charging on food prep table.

13 Containers of raw meat were stored on a shelf above containers of cooked food. Moved.
**Raw meats should always be stored under cooked and ready to eat food.
20 **The Coca Cola refrigerator is holding salsa and flan at 46 degrees F instead of 41 degrees For below.
**Food in the cold prep refrigerator was held at 48 degrees F rather than 41 degrees F or below as required. A container of chicken dorado was discarded.
34 **A thermometer is not provided in the Coca Cola refrigerator.
**Staff does not have a probe thermometer to take temperatures of food items.
36 Visible physical evidence of insect activity observed in the * area.
**One german roach was seen in the prep area
