13 Raw meats observed stored above par cooked /heat and serve meats in refrigerated drawer on cooks line.
13 Designated food preparation sink observed directly plumbed. A food prep sink should be indirectly plumbed with a visible air gap.
**Please note prep sink cannot be used as a handwashing sink.
14 Employee observed washing dishes without sinks set up to properly wash, rinse and sanitize.
Corrective Action: The food safety certified person intervened and set up sinks properly. Sanitarian went over the proper Wash- Rinse- and Sanitize procedure.
14 The can opener had an accumulation of food residue on the food contact surfaces.
21 Opened commercially processed ready to eat food, located in the reach in refrigeration unit, and held more than 48 hours, was not marked with the date it was opened/cut.

47 The exterior of bulk food ingredients were observed with residue/debris accumulations.
48 Slow hot water generation observed in toilet room.
51 -Floor/wall junctures are not tightly sealed with a coved finish in toilet room.
-Ceiling vent observed with dust accumulation in toilet room.

51 A separate covered trash receptacle for sanitary napkins were not observed in toilet room.
53 -Open utility penetrations observed in the rear prep area near the handwashing sink and in rear storage are near toilet rooms.
-Open ceiling surfaces observed in rear storage area.
-Open wall observed on the inner wall of basement along the stairs and in prep area with pipes protruding through walls.
-Damaged floors observed below handwashing sink in prep area.
-Cracked flooring and missing coved finishes observed in rear prep areas.
53 -Debris accumulation observed along floor perimeters in warewash area/food prep area.
-Residue accumulations observed on floors and wall in grease storage area.
-Dirt accumulation observed on basement floors.
-Mouse droppings observed on floor in corner by handwashing sink.
-Old equipment observed but is not in use: Ice cream machine
53 Idle maintenance tools observed sored on floor instead of elevated onto utility rack.
58 New floors observed in customer area and cooking -prep area.

65 Food establishments producing grindable garbage waste must install a garbage grinder to properly dispose of such waste or recycle the garbage by other approved means. Dumpsters may not be used for the disposal of grindable garbage.
