1 Observed food safety person with ServSafe Training Certificate. No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. **Health Sanitarian emailed Phila. Food Safety Certificate application 03.17.24 to PIC**
8 No paper towel or hand dry device located near the handwash sink in the Children's Toilet room.
13 Observed a non-NSF/ANSI microwave located on domestic counter-top of handwash sink, no splash guard installed between the handwash sink and microwave to prevent potential contamination of open food.
14 The following observations were noted: -Newly installed domestic sink in the infant-3 yrs. old classroom used to dump and unapproved washing process of children sippy cups on site -Multi-use bakeware and serving utensils (i.e. baking pan, cutting board, can opener, tong, ice cream, scooper & measuring cups) stored in the kitchen area; no 3-compartment warewash sink installed to properly wash, rinse and sanitize the bakeware, sippy cups and serving utensils. **Health Sanitarian advised person in charge that a warewash sink is required, in order to properly wash and sanitize multi-use utensils and children drinkware. **Childcare establishment is only approved to use single use utensils, tableware and bakeware. Childcare is required to inform children parents to provide more than 1 drinkware and/or baby tableware if children are at the establishment for more than 4 hours.

49 The following observation were noted: -No food prep sink installed **Childcare facility is only approved to use and serve already pre-washed, pre-sliced and/or canned(with peel back lids) fruits and vegetables.** -No 3-compartment warewash sink observed **Health sanitarian instructed person in charge to discontinue using multi-use cookware and serving utensils; and discontinue washing children's sippy cups for re-use.**
61 Initial Plan Review approval stated parents were to provide food for the children. As of today, food facility is providing food to enrolled children, observed a non NSF/ASNI microwave and conventional oven used reheat food, and prepackage food stored on the food storage rack. **Food establishment licensee and operation changes require submission of a Plan Review Application, included food facility food menu and a NSF/ANSI Reheating appliance. 65 No garbage grinder observed. **Installation of a 1/2 horsepower garbage grinder is needed.** {Food establishments producing grindable waste (i.e. Produce-vegetables/fruits) must install a garbage grinder to properly dispose of such waste or recycle the garbage by other approved means. Dumpsters may not be used for the disposal of grindable garbage.}

6 Food employees observed donning single use gloves without a prior hand wash in the food prep area. **Corrective action was taken, food employee observed removing single use gloves and washed hands at time of inspection.**
8 Observed food residue, multi-use food containers, bakeware and utensils stored in the handwash sink in the food prep area. (2) A handwashing facility may not be used for purposes other than handwashing. **Corrective action was taken, person in charge removed the multi-use cookware and serving items, cleaned out the sink at time of inspecction.**
14 Observed multi-use bakeware, food containers and serving utensils being used but, no 3-compartment warewash sink installed to proper wash, rinse and sanitize the bakeware, food containers and serving utensils. **Health Sanitarian advised person in charge that a warewash sink is required, in order to properly wash and sanitize multi-use utensils.
