14 Slicer has an accumulation of food residue on the food contact surfaces and is not being washed, rinsed and sanitized at least every 4 hours.
20 The following observations were noted: -Cooked pasta was held on the counter at 60 degrees F rather than 41 degrees F or below as required. -Cooked rice was held in the rice cooker at 58 degrees F rather than 41 degrees F or below as required. PIC took corrective action and discarded the rice and pasta during inspection.
21 No dates observed on food in the walk-in cooler, under-counter refrigerator, and reach-in refrigerator.

47 - Grease accumulation and residue observed behind the cooking equipment in the food prep area. -Debris observed on the floor perimeters in the storage area.
49 An approval airgap was not observed for the designated food prep sink.
49 The following observations were noted: -No utility sink observed in the facility. -No backflow prevention device observed on the main water inlet. -Slow water drainage observed at the hand wash sink in the food prep area. -Pipe was leaking underneath the prep sink, 3 bay sink, and hand wash sink in the cooking area.
