8 Paper towel dispenser empty at the handwash sink in the restroom area. Paper towel was replenished during the inspection.
11 Severely dented, swollen, distressed canned items observed in retail shelf area. Dented canned item was removed from retail shelf during the inspection.
14 The quarternary ammonia concentration in the sanitizing solution of the 3-bay warewash sink was 150 ppm, rather than 200 - 400 ppm as stated on the manufacturer's use directions. Additional QAC pills added to solution to derive 200 ppm during the inspection.
36 Side entrance garage door to storage area outer opening in the food facility does not protect against the entry of insects, rodents, and other animals.
45 Cutting board observed with deep scratches not allowing for effective cleaning and sanitizing in meat prep area.
