Application was emailed.

1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. **Person in charge /food safety certified person does not have a valid food safety certificate. Food safety certificate expired 4/29/24. Person in charge has provide receipt for food safety course taken on 4/29/24.
20 Several foods were held at 52.3 to 55.9 degrees F rather than 41 degrees F or below as required. Food items were relocated to walk-in cooler. The ambient air of the reach-in refrigerator was observed at 45 degrees F. Please discontinue usage until a proper temperature of 41 degrees F can be maintained.

Food safety certified person (Leonel Galeano) arrived about 20 minutes into the inspection.
