14 The food-contact surfaces of equipment and utensils did not reach 160 degrees F during the sanitizing cycle of the high temperature dishwasher as evidenced by the failure of the temperature indicator. Facility to use the 3-compartment warewashing sink until the dishwasher is serviced and working properly. An emergency work order was placed for the dishwasher.
19 Potentially hazardous food (burgers, meatloaf) was held at 129-130 degrees F rather than 135 degrees F or above as required. Unit temperature was adjusted.
20 Potentially hazardous food (eggs, cheese, spinach) was held in the refrigerator in the cafeteria (lunch line) at 50-54 degrees F rather than 41 degrees F or below as required. Foods were removed from the unit to be discarded.
45 The following were observed: -Hot holding unit located in cafeteria area does not maintain proper hot holding temperature. Ambient air observed at 127 degrees F rather than 135 or higher as required. -Reach-in refrigeration unit located in cafeteria area does not maintain proper cold holding temperature. Ambient air observed at 55 degrees F rather than 41 or below as required.

Ambient air observed at 45-57 degrees F rather than 41 or below as required. **All potentially hazardous food observed within proper temperature ranges. PIC was instructed to move any potentially hazardous foods to units that maintain proper cold and hot holding temperatures until serviced by maintenance.

1 Food Safety Certified person's city certificate observed expired. **No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. **PIC was issued application for City Certificate during time of inspection.
11 One gallon of milk located in the walk-in cooler observed with "Sell By" date 11/30/22. **PIC took corrective action and discarded milk during time of inspection.
14 The following were observed: -Slicer has an accumulation of food residue on the food contact surfaces and is not being washed, rinsed and sanitized at least every 4 hours -Pink slime observed forming on the interior of the ice machine bin **PIC took corrective action and discarded ice, and properly washed, rinsed, and sanitized food contact surfaces of ice machine/slicer during time of inspection.
26 Open rodent bait observed under ware-washing sink in food prep area. **PIC took corrective action and discarded open rodent bait. **Rodent bait shall be contained in a covered, tamper-resistant bait station.
36 An insect control device (glue tras) observed stored above 3 bay ware-washing sink located in food prep area with potential to contaminate food equipment, and / or utensils. **PIC took corrective action and relocated glue trap during time of inspection.
43 Single use articles located in dry storage area observed improperly inverted. *PIC took corrective action and properly inverted single use articles during time of inspection.

14 The quaternary ammonia concentration in the sanitizing solution of the 3-bay ware-washing sink was greater than the required range as stated on the manufacturer's use directions. **PIC was instructed to discontinue use of 3 bay ware-washing sink until serviced by maintenance and to utilize the mechanical high temp ware-washing machine.
19 Potentially hazardous food located in hot holding unit was held at 101 degrees F rather than 135 degrees F or above as required. **PIC was instructed to discard all potentially hazardous foods out of required temperature range.
20 Potentially hazardous food located in open display case was held at 57-61 degrees F rather than 41 degrees F or below as required. **PIC was instructed discard potentially hazardous food out of temperature range during time of inspection.
