11 Severely dented/distressed canned items observed on the shelves in the retail area.
**The affected cans were discarded at the time of the inspection.
14 The quaternary ammonium concentration in the sanitizing solution of the 3-bay warewash sink was 0-100 ppm, rather than 200 ppm as stated on the manufacturer's use directions.
**Additional tablets and hot water was added to the solution to increase the concentration.
21 Opened commercially processed ready to eat foods (Deli meat & cheese), located in the display case, and held more than 48 hours, was not marked with the date it was opened.
26 Observed insecticides/rodenticides (Bug spray & loose rodent bait) not labeled by the manufacturer for food facility usage, in the prep and retail area.
26 Chemicals (bug spray) was stored on the same shelf as food equipment in the prep area.
**The can of bug spray was relocated at the time of the inspection.
36 Visible physical evidence of rodent and insect activity observed in the basement and upper side area.

21 Potentially hazardous ready to eat food, repackaged in the food facility and held for more than 48 hours was not date marked: packaged mayonnaise based salads (potato, macaroni, chicken) were not labeled with the common name of food items and date. *PIC provided proper labels.
27 Food facility was packaging foods using reduced oxygen method without the required written procedures or HACCP plan: 1 package of frozen pork sausage observed in retail freezer. *ROP process is not approved for this facility. PIC removed food item to properly discard.
36 Outer openings are not protected from rodent/insect entry: Small gaps observed around basement window (west-facing).
37 Consumer self-service tongs for pickles were not properly stored to prevent contamination. *PIC relocated tongs.
45 Equipment does not meet NSF or ANSI-Sanitation standards: raw wood shelf in garage storage area (beverage storage).

49 An approved indirect waste drain connection is not provided: drain line from walk-in cooler is directed to the floor.
