1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. Application emailed.
8 -No sign or poster posted at the handwash sink in the front ice cream area to remind food employees to wash their hands. Person in Charge took corrective action and provided signage.
-Paper towel dispenser empty at the handwash sink in the warewash area. Corrective action was taken and paper towels were provided.
11 Expired apple cider observed in bar area. See discard list.
14 Pink slime observed on the interior of the soda gun holster. Person in Charge took corrective action and cleaned and sanitized surfaces.
18 Lettuce prepared from food ingredients held at ambient temperature not cooled to 41 degrees F within 4 hours as required. Person in Charge took corrective action and relocated foods to bring food temperatures down.
20 Cut lettuce, spring mix, and cut tomatoes as held 46 and 56 degrees F rather than 41 degrees F or below as required. Spring mix was discarded. Cut lettuce and cut tomatoes were relocated to bring food temperatures down.

1 The food facility operator shall be the Person in Charge or shall designate a Person in Charge and shall ensure that a Person in Charge is present at the food facility during all hours of operation. Person in Charge arrived during the time of inspection.

-Cream cheese, and milk, held between 49-57 degrees F, rather than 45 degrees F or below as required. Cream cheese were discarded and Milk were relocated. Cold hold unit was taken out of service.
