21 Multiple potentially hazardous ready to eat food items (ham, sauces) prepared in the food facility and held for more than 48 hours, located in the under-counter refrigeration unit in rear prep area- were not date marked.
36 Visible physical evidence of rodent activity observed:
-mouse droppings observed in the rear dry storage area
-mouse droppings observed along floor perimeters in utility closets
-mouse droppings observed along floor perimeters in the warewashing area
37 Beverages are not stored at least 6 inches above the floor in the prep area.
37 Prepped food (cooked bacon) observed sitting on the prep area counter is in close proximity to the handwash sink and the handwash sink lacks a splash guard.

1 The food facility operator shall be the Person in Charge or shall designate a Person in Charge and shall ensure that a Person in Charge is present at the food facility during all hours of operation.

8 The handwash sink was blocked by a trash can in the front prep/ service area. A handwashing facility shall be maintained so that it is accessible at all times for employee use.

14 Ice machine in the rear prep area was observed with heavy food residue accumulation on the inside with the ability to contaminate the ice.
20 Multiple food items (whole milk, half & half, soy milk) were held at 54-55 degrees F rather than 41 degrees F or below as required. The ambient temperature of the under-counter refrigeration unit was 55 degrees.
*Corrective action: These items were relocated to a colder unit at the time of inspection as they were just delivered today- other items were kept there that are not potentially hazardous (almond milk, coconut milk). PIC was directed discontinue use of under counter unit until properly serviced*
20 Two large containers of cream cheese were observed being held at 45-46 degrees F rather than 41 degrees F or below as required in the under-counter refrigeration unit (under the toasters area). The ambient temperature of the unit was 46 degrees. *PIC relocated cream cheese to a colder unit as it was just prepped within 3 hours; and discontinued use of unit*
21 Multiple potentially hazardous ready to eat food items (large tubs of cream cheese, sauces) prepared in the food facility and held for more than 48 hours, located in the under-counter refrigeration unit in rear prep area- were not date marked.
34 Temperature measuring device in the under-counter refrigeration unit at front coffee/ service area was defective. Was reading unit was 32 degrees; however, ambient air temp was 55 degrees, and all probed food items stored in unit were ranging between 54-56 degrees F.
