14 Black residue and debris observed on the interior of the ice machine bin around baffle plate. Corrective action was taken and ice was burned and surfaces washed, rinsed and sanitized.
35 Working bulk food ingredient storage containers are not labeled with the common name of the food.
45 -Milk crates observed being used for elevation is not NSF/ANSI approved.
-Walk-in freezer is not in good repair. Bottom exterior of walk-in freezer was not in good repair.
47 -Accumulation of dust on milk crates.
-Residue observed along surfaces of under-counter door.

14 A knife and other utensil were observed being stored in a container of sanitizer between uses. Utensils may not be stored in sanitizer and used directly on food items, without properly air-drying.
Facility was directed to discontinue method of storing utensils between uses.

45 - The kitchen handsink and dishmachine drainboards were not properly sealed to the wall.
45 Aluminum foil was observed being used as lining in the kitchen area.

53 Broom and dustpan was observed being stored on the floor between uses; not stored on a utility rack as required.

1 The Person in Charge did not demonstrate adequate knowledge of the Philadelphia Food Code. Person in Charge did not know the 5 food borne illnesses. A copy of the employee health policy was emailed.
12 Shellstock located in the walk-in cooler did not have identification tags attached to the container. Person in Charge provided tags
14 Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was 0 ppm, and not 50-100 ppm as required. Discontinue dishwasher operation until fixed.
20 Burgers was held at 44-47 degrees F rather than 41 degrees F or below as required. Burgers were broken down into smaller containers and relocated to walk-in cooler to bring food temperatures down.
47 -Accumulation of ice observed in chest freezer.

1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. Application emailed.
13 -Raw eggs were stored above ready to eat foods in the walk-in cooler. Person in Charge relocated items.

51 Lidded trash receptacle for disposing feminine hygiene products were not observed.
53 -Old mouse droppings observed on the floor near ice machine, under shelving unit and dry storage area in the basement.
