13 -Raw chicken was stored above raw beef, pork, fish in the under counter fridge and walk-in cooler. -raw pork, beef, and chicken were stored together in the cold prep fridge and observed without any coverings. *PIC corrected on site*
14 The chlorine concentration in the sanitizing solution of the 3-bay warewash sink was 0 ppm, rather than 50-100 ppm as required. *PIC corrected sanitizer on site*
21 Multiple potentially hazardous ready to eat food items, prepared in the food facility and held for more than 48 hours, located in the under counter fridge, reach in fridge and walk in cooler, were not date marked.
26 Working containers in the warewashing area, used for storing chemicals taken from bulk supplies, were not marked with the common name of the chemical.

Dead rodents shall be removed from control devices and the premises at a frequency that prevents their accumulation, decomposition or the attraction of pests.
