13 Food observed uncovered in the reach-in freezer and reach-in refrigerator (fries and churros) from the prep area.
14 The can opener had an accumulation of food residue on the food contact surfaces.
20 Food (cheese and red sauce) was held between 52 degrees F and 55 degrees F rather than 41 degrees F or below as required. PIC removed the cheese and red sauce from cold prep unit to reach-in refrigerator.
36 Visible physical evidence of rodent (mouse droppings) observed inside the cabinet in the service area. Live cockroaches observed behind the ice machine.
