47 The following were observed: -Food debris and residue around perimeters of floors in all walk-in coolers and walk-in freezers. -Residue and dust inside and outside on gaskets / reach-in refrigeration unit (with soda) retail area. -Food debris observed on the floor aisle 3 and 4.

8 The following were observed: -Paper towel dispenser empty at the handwashing sink in the rear storage area -Hot water does not reach peak demand at handwashing sink in the unisex employee restroom. Hot water observed at 64 degrees F rather than 100-110 degrees F as required. **PIC took corrective action and provided paper towel. PIC was instructed to close unisex employee restroom and utilize other unisex employee restroom where hot water reaches peak demand until serviced by maintenance.
8 The handwashing sink in the rear storage area was blocked by chemical bottle and not accessible at all times for employee use. **PIC was instructed to store chemicals in a chemical only designated area during time of inspection.
11 The following were observed: -Several dented cans in canned foods aisle -Visible evidence of rodent contaminated foods (gnawed chip bag) in the chip aisle area **PIC was instructed to discard all adulterated foods during time of inspection.
36 Visible physical evidence of rodent activity (mouse feces) observed in the rear storage room area near walk-in cooler.

14 The quaternary ammonia concentration in the sanitizing solution of the 3-bay ware-washing sink in the Bakery Department was 0-150 ppm, rather than 200-400 ppm as stated on the manufacturer's use directions. **PIC was instructed to have sanitizer solution serviced by EcoLab to increase concentration.
