3280.106 A facility shall have an operable, clean refrigerator (maintaining food at 45°F or below with an operating thermometer) used to store potentially hazardous foods.
    Ambient air of the refrigerator measured at 48.7 degrees F and cheese was measured at 51.8 degrees F.
Emergency Contact Information
3280.124(a) Emergency contact information is required for each enrolled child. Emergency contact information must reference who shall be contacted in an emergency.

Adult Hygiene
3280.152 A facility person shall wash his hands before meals and snacks, after toileting and after diapering a child.
    Paper towels observed not available at the handwash sinks in the restroom or food prep areas.
General Health and Safety
3280.20 Conditions at the facility may not pose a threat to the health or safety of the children.
    -Food contact surfaces observed not properly sanitized. -Mouse droppings observed in the facility. -Reach in refrigerator observed not holding cold foods at the proper temperatures.
Protective Electrical Covers
3280.65 Protective electrical covers shall be placed in electrical outlets accessible to children 5 years of age or younger.
    Observed uncovered electrical outlets in the main room and sitting areas. Corrective Action: Outlet covers were placed on the electrical outlets.
Toxics
3280.66(b) Cleaning and other toxic materials shall be used so as not to constitute a hazard or contaminate play surfaces, food, and food preparation areas.

3280.75(c) A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.
