    Observed emergency contact forms in all rooms that have not been updated once in a 6 month period.
Health and Safety
3270.21 Conditions at the facility pose a threat to the health and safety of the children.
    Observed mouse droppings around the floor perimeter of rooms YOT-1 & YOT-2, the children's restroom and the transitional room (currently not in use).
First-Aid Kit
3270.75(c) A first-aid kit does not contain at least the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.
    - Observed first aid kit missing tweezers in OLT room. - Observed first aid kit missing a bottle of water in Jr K-S/A room. **A pair of tweezers and a bottle of water was supplied to the kits at the time of the inspection.
Physical Environment
3270.76 Floors, walls, ceilings, and other surfaces are not kept clean, in good repair and free from visible hazards.

This inspection has revealed that the establishment is not in satisfactory compliance and that current management practices have allowed unacceptable public health conditions. Corrective action is required to eliminate these violations. Compliance status will be assessed upon reinspection.
