VIOLATIONS

Play equipment 3270.102(a) Toys, play and other equipment are not clean and in good repair.     Infant toys observed on the floor (infant room). Child Records 3270.181(a), (b) An operator has not established and maintained current, individual records for each child enrolled in the facility.     Emergency contact number not updated in the infant room. Health and Safety 3270.66(b) Cleaning and other toxic materials are not properly labeled and stored away from child care spaces.     Spay bottle in kitchen area, used for storing chemicals taken from bulk supplies, were not marked with the common name of the chemical. Sanitation 3270.67(c) Evidence of insect or rodent infestation observed in the facility.     Mouse droppings observed on the floor in the infant room and on the window shelving and floor in the kitchen area. Physical Environment 3270.76 Floors, walls, ceilings, and other surfaces are not kept clean, in good repair and free from visible hazards.     Mouse droppings observed on the floor in the infant room. Gaps in the ceiling in the infant room.       General Comments: Out of compliance.

This inspection has revealed that the establishment is not in satisfactory compliance and that current management practices have allowed unacceptable public health conditions. Corrective action is required to eliminate these violations. Compliance status will be assessed upon reinspection.
