VIOLATIONS

Furniture and Rest Equipment 3270.104(a) Furniture is not durable, safe, easily cleaned and appropriate for the child's size, age and special needs.     Play shelf in the Olympia room was not anchored. Child Records 3270.181(c) A guardian has not updated the emergency contact information at least once in a 6-month period.     Multiple emergency contact documents were dated 2021. Physical Environment 3270.63 Fencing or natural barriers are not provided to restrict children from unsafe areas or conditions.     Screen panel in the outdoor play area used to block access to a set of stairs had a large tear in the screen. Toilet Area 3270.82(j) A source of running water for hand washing is not present in infant and toddler diapering areas.     -No source of running water is present in the parish room where a changing table is present. -Portable sink in the Olympia room was out of order. Health and Safety 3270.92(c) A fixed space heater is not insulated or equipped with protective guards.     Radiators in the parish room are not insulated or equipped with a protective guard.

This inspection has revealed that the establishment is not in satisfactory compliance and that current management practices have allowed unacceptable public health conditions. Corrective action is required to eliminate these violations. Compliance status will be assessed upon reinspection.
