VIOLATIONS

Child Records 3270.181(c) A guardian has not updated the emergency contact information at least once in a 6-month period.     Emergency contacts observed not being updated every 6 months. Health and Safety 3270.66(a) Cleaning and other toxic materials are accessible to children.     -No lock observed stored on chemical/sanitizing cabinet in classroom area. -No lock observed on kitchen cabinet. First-Aid Kit 3270.75(c) A first-aid kit does not contain at least the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.     1 first aid kit observed in need of restocking. Physical Environment 3270.76 Floors, walls, ceilings, and other surfaces are not kept clean, in good repair and free from visible hazards.     Damaged ceiling surface observed in front and rear classroom area. Toilet Area 3270.82(g) Toilet areas and fixtures are not cleaned maintained in good repair.     - Unnecessary clutter and maintenance tools observed in 2nd restroom. - Dust accumulation on ceiling vents in restroom

This inspection has revealed that the establishment is not in satisfactory compliance and that current management practices have allowed unacceptable public health conditions. Corrective action is required to eliminate these violations. Compliance status will be assessed upon reinspection.
