Protective Electrical Covers
3270.65 Protective electrical covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Toxics
3270.66(b) Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content.
    Working containers in Classroom #13, used for storing chemicals taken from bulk supplies, were not marked with the common name of the chemical. Chemicals were stored above or on the same shelf with and/or single service articles in Classroom #8. **PIC took corrective action and labeled chemicals/moved chemicals to a chemical only designated area during time of inspection.
Sanitation
3270.67(c) Evidence of infestation of insects or rodents in the facility is not permitted.
    The following were observed: -Potential rodent harborage areas inside the food facility observed in the classroom areas and utility closets due to unused supplies/material. -Visible physical evidence of rodent activity (mouse feces) observed around perimeters of floors in the following areas: -Classroom #12 (restroom, closets, and on exterior surfaces of shelving units where art supplies are stored) -Classroom # 9 (in corner near handwashing sink and closets) -Classroom #11 (in closets) -Classroom #6 (in restroom area) -Classroom #1 (on lower interior surfaces of cabinetry unit, on exterior surfaces of step stool, and in closets)
Water
3270.69(b) Hot water temperature, in areas accessible to children, may not exceed 110° F.
    The following were observed: -One to two minute delay in hot water reaching peak demand in Classrooms #7 and 9 -Low pressured water in Classroom #9 -Water does not reach peak demand at handwashing sinks in Classrooms #1,2,5, and kiddie restroom in rear of facility. Hot water observed between 90-97 degrees F, rather than 100-110 degrees F as required.; Repeat violation
Indoor Temperature
3270.70(a) The indoor temperature must be at least 65° F.
    PIC CONFIRMED THAT FACILITY HEATER IS BROKEN. AMBIENT AIR TEMPERATURE BETWEEN CLASSROOMS #1, 2, 3, 4, AND 5 OBSERVED BETWEEN 50-60 DEGREES F AT DOOR, MIDDLE OF ROOM, AND WINDOWS. CLASSROOMS ARE CURRENTLY BEING COMBINED UNTIL HEATER IS SERVICED BY MAINTENANCE.

3270.73 A facility shall have an operable telephone and a published telephone number.
    Telephones/walkie talkies were not observed available in the following classrooms: -Classroom #9 -Classroom #7 -Classroom #8 -Classroom #6 -Classroom #3 -Classroom #2 -Classroom #1
Emergency Telephone Numbers
3270.74 The telephone number of the nearest hospital, police department, fire department, ambulance and poison control center shall be posted by each telephone in the facility.
    Emergency contact numbers were not observed posted in the following Classrooms: -Classroom #5 -Classroom #12 -Classroom #8 -Classroom #11
Building Surface Requirements
3270.76 Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.
    The following were observed: -Porous stained ceiling tiles -Open holes throughout facility -Base coving needed around perimeters of floors in some restrooms and closets; Repeat violation
Lighting
3270.78 Rooms, hallways, stairways, outside steps, porches and ramps shall be lighted by artificial or natural light.
    One light out in Classroom #14.
This inspection has revealed that the establishment is not in satisfactory compliance and that current management practices have allowed unacceptable public health conditions. Corrective action is required to eliminate these violations. Compliance status will be assessed upon reinspection.
