PM-305.1 The interior of a structure and equipment therein shall be maintained in good repair, structurally sound and in a sanitary condition.
    Dusty ceiling tiles and ceiling vents observed in 1st floor learning center.- Inoperable washer & dryer in Learning center restroom. - Dust accumulation on hallway ceiling vent in front of 1st floor learning center.- Toilet bowl lid observed missing in Room 403 restroom. - Ceiling Sprinkler cap missing in suite 404. Room 404 wall and ceiling observed peeling with ceiling water damage. - Room 445 restroom wall surface peeling. Room 545 Electrical wall socket in hallway observed loose. - Suite 547 has damaged wall surface. -Room 533 faucet observed loose. Room 521 electrical socket not properly sealed to wall Surface. Suite 649 has visible floor gap along wall perimeter. Suite 637 has wall damage behind door. - Art centers observed not in use. 5th floor art center is used for storage.; Repeat violation

This inspection has revealed that the establishment is not in satisfactory compliance and that current management practices have allowed unacceptable public health conditions. Corrective action is required to eliminate these violations. Compliance status will be assessed upon reinspection.
