| Observations: -Rodent droppings observed on the floor perimeters in classrooms closets and the basement in the rooms near the hot water heater. -Dust and debris on the floor perimeters in the basement areas near the hot water heater and basement classroom closets. -Black residue and dust and debris on the second floor Black classroom wall and floor perimeters in the corner below the heater. -Terro ant bait observed in a classroom closet. This area was cleaned during the inspection. -Open poison blocks observed on the floor in the room near the hot water heater. | |

| This inspection has revealed that the establishment is not in satisfactory compliance and that current management practices have allowed unacceptable public health conditions. Corrective action is required to eliminate these violations. Compliance status will be assessed upon reinspection. |
