Condition of Play Equipment
3270.102(a) Toys and play equipment must be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.
    Large toys and stacked plastic crates are not anchored to prevent tipping in classrooms.
Refrigerator
3270.107 A facility shall have an operable, clean refrigerator (maintaining food at 45°F or below with an operating thermometer) used to store potentially hazardous foods.
    Surfaces in the basement commercial refrigerator were soiled.
Toxics
3270.66(c) Cleaning and other toxic materials shall be used so as not to constitute a hazard or contaminate play surfaces, food, and food preparation areas.
    Cleaning solutions stored next to or over food in classroom closet areas. The observed items were moved. Corrected On Site
Sanitation
3270.67(c) Evidence of infestation of insects or rodents in the facility is not permitted.
    Visible physical evidence of rodent activity (rodent droppings) observed in the classroom closets and basement hot water heater areas.
Ventilation
3270.72(a) Natural or mechanical ventilation shall be provided in child care spaces.
    The second floor toilet room does not have an exhuast fan or window.
Building Surface Requirements
3270.76 Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.
    Observations: -Rodent droppings observed on the floor perimeters in classrooms closets and the basement in the rooms near the hot water heater. -Dust and debris on the floor perimeters in the basement areas near the hot water heater and basement classroom closets. -Black residue and dust and debris on the second floor Black classroom wall and floor perimeters in the corner below the heater. -Terro ant bait observed in a classroom closet. This area was cleaned during the inspection. -Open poison blocks observed on the floor in the room near the hot water heater.
Paint
3270.77(a) Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.
    -Wall and floor damage in the second floor Black classroom. -Openings in lower wall floor junction areas in a secocnd floor classroom closet, the basement classrooms and staff break room areas. -Peeling paint in the area leading to the hot water heater area.
   
   
   
   
   
   
   
   
   
This inspection has revealed that the establishment is not in satisfactory compliance and that current management practices have allowed unacceptable public health conditions. Corrective action is required to eliminate these violations. Compliance status will be assessed upon reinspection.
