Child Hygiene
3270.134(c) Paper towels may be used as towels and washcloths. Paper towels shall be discarded after each use.
    -Paper towels not observed in toilet room. *PIC provided paper towels during inspection.
Diapering Requirements
3270.135(a)(3) Soiled diapers shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can.
    -Trash receptacle in diapering area not hands free.
Minimum Number of Facility Persons in the Child Care Facility
3270.54(a) At least two facility persons shall be present in the facility when two or more children are in care.
    -Child to staff ratio was observed at 1:7 during the beginning of inspection. *A staff person arrived 20 mins after start of inspection.
Sanitation
3270.67(c) Evidence of infestation of insects or rodents in the facility is not permitted.
    -Mouse droppings observed on top of cubbies, in chairs, in storage closets, in storage cube bins in infant room, on strollers, on pillows in infant area. -Gnawed on bag in cube bin in infant room.
Emergency Telephone Numbers
3270.74 The telephone number of the nearest hospital, police department, fire department, ambulance and poison control center shall be posted by each telephone in the facility.
    -Emergency telephone numbers not posted near phone. *PIC provided phone numbers during inspection.
First-Aid Kit
3270.75(c) A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.
    -First aid kit missing gauze tape in front class room.
Building Surface Requirements
3270.76 Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

This inspection has revealed that the establishment is not in satisfactory compliance and that current management practices have allowed unacceptable public health conditions. Corrective action is required to eliminate these violations. Compliance status will be assessed upon reinspection.
