| General Comments: Establishment does not presently use sky deck play area as it requires repairs. 2 of the rooms in the facility are not actively used and a children's restroom in a infant room is used for storage as the children housed in it are not potty trained. This establishment has not had a food license since 2015. AN LER cannot be issued as there is no present FSC personnel on site. An LER can be issued after a FSC is present and past due food license fees are paid. | |
