3270.106(a) Individual, clean, age-appropriate rest equipment shall be provided for children and must be labeled for and used by a specific child.
    Rest equipment (cots) observed without name labels in Older Toddler Classroom. **PIC was instructed to label all rest equipment (cots) with names of students during time of inspection.
Child Hygiene
3270.134(a) A staff person shall ensure that a child's hands are washed before meals and snacks, after toileting and after being diapered.
    The following were observed: -No running hot or cold water observed at the handwashing sink in the Pre-K Classroom -Hot water does not reach peak demand at handwashing sink in Early Toddlers Classroom. Hot water observed at a temperature of 90 degrees F and below.
Adult Hygiene

    Safety locks not provided on cabinetry units of handwashing sinks the Older Toddler Classroom and Pre-K Classroom.
Protective Electrical Covers
3270.65 Protective electrical covers shall be placed in electrical outlets accessible to children 5 years of age or younger.
    Two electrical outlet covers not observed in food prep area. **PIC took corrective action and provided electrical outlet covers during time of inspection. (COS)
Sanitation
3270.67(c) Evidence of infestation of insects or rodents in the facility is not permitted.
    Visible physical evidence of rodent activity (mouse feces) observed on lower interior surfaces of handwashing sink's cabinetry unit in the food prep area.
Water
3270.69(b) Hot water temperature, in areas accessible to children, may not exceed 110° F.

Emergency Telephone Numbers
3270.74 The telephone number of the nearest hospital, police department, fire department, ambulance and poison control center shall be posted by each telephone in the facility.
    Emergency telephone numbers were not observed posted in the Older Toddlers Classroom. **PIC took corrective action and provided Emergency telephone numbers during time of inspection. (COS)
First-Aid Kit
3270.75(c) A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.
    First aid kit located in the Infant Classroom observed without a bottle of water. **PIC took corrective action and provided water bottle during time of inspection. (COS)
This inspection has revealed that the establishment is not in satisfactory compliance and that current management practices have allowed unacceptable public health conditions. Corrective action is required to eliminate these violations. Compliance status will be assessed upon reinspection.
